Ulster County Vaccination Clinic at Phoenicia School February 1, 2022

The Clinic will be from 4:00 - 6:00 pm. Please click on the appropriate link to register you or your child - depending on the age and the vaccine that you want.

  • After you register, you will see a ticket, please bring  this with you
  • Appointments are required
  • Children must be accompanied by their parent/legal guardian
  • You must have Photo ID
  • Please do not arrive prior to 15 minutes before your scheduled appointment
  • Prior to your appointment, please complete the NYSDOH COVID-19 Vaccine Form you will see on the registration screen; please bring proof of completion to your appointment
  • You must bring your COVID-19 vaccination card/proof of vaccination to your appointment

Come in the front, follow signage to Gym. 

Second doses/Boosters will be given on March 1st.
