Before you begin the registration form you will need the following information ready to enter/upload: (You can scan or take a picture with your phone and upload it)
| Household information - address and phone numbers |
| Proof of Residency |
| Parent information - home, cell and work numbers; email addresses |
| Emergency Contact - addresses and phone numbers |
| Student information - Proof of Age, most current Report Card, medical info, |
| Proof of Immunization and a current physical (within 1 year) |
| Most current Individualized Education Plan (IEP) or 504 Accommodation Plan |
| Documentation relating to Special Circumstances - Custody/Visitation agreements OR if you are not the natural parent but have legal guardianship of the student, please provide any available relevant Court documents |
